Learn how to prevent diabetes end stages at a diabetes training center such as the Diabetes Outpatient Clinic offered at University Hospital in Columbia. The center features exam rooms, classrooms for patient education and a model kitchen used to teach meal planning.
Medical team members at the Cosmopolitan International Diabetes Center work to help patients with diabetes achieve the following:
- Hemoglobin A1c less than 7 percent
- Blood pressure less than 130/85
- Total cholesterol less than 200 mg/dL
- HDL (“good” cholesterol) greater than 35 mg/dL
- LDL (“bad” cholesterol) less than 130 mg/dL
- Triglycerides less than 400 mg/dL
During the first visit, patients meet with a dietitian to discuss nutritional meals and map out a meal plan. Patients then visit with a nurse educator to learn about diabetes and develop an individual treatment plan to care for the disease comprehensively. Patients finally meet with a doctor to set goals and discuss any other issues. The entire process may take up to four hours.
Patients are evaluated on how well they’re following their treatment plans in follow-up appointments three to four times per year.
Patients can learn more on their own through The Cosmopolitan International Diabetes Center Patient Library. The library contains a selection of books, videotapes and audiotapes on a variety of diabetes-related topics. It also includes a large number of cookbooks. These are all available for patients to check out at no charge.
